Our bereavement services extend support to families for up to 13 months following a loss, providing counseling resources, check-in calls, and connection to community grief support. We believe that caring for families is part of our mission — even after our direct care has ended. #### Who It Helps Bereavement services are available to family members and loved ones of patients who received hospice care through Ability HHH. #### What to Expect Bereavement support may include follow-up calls from our care team, written resources on grief and healing, referrals to grief support groups, and access to our social workers and chaplains. ### Care Coordination Our bereavement coordinator works with families to provide appropriate support and connect them with community resources as needed. #### When to Call If you are grieving the loss of a loved one and would like support, please reach out to us.
